# OpenClaw Skill: OB-GYN Prenatal and Well-Woman Coordinator
## Overview
This skill turns the OpenClaw agent into a prenatal and well-woman coordinator
for an OB-GYN practice. It owns the ACOG prenatal visit cadence, the timing
windows for NIPT, the first-trimester screen, the anatomy scan, the 1-hour
glucose challenge, the 36-week GBS swab, the postpartum 6-week visit, and the
annual well-woman recall keyed to the patient's last Pap or HPV co-test and
last mammogram.
It is designed for 1 to 6 provider OB-GYN practices running athenaPractice
(athenahealth), ModMed OBGYN, eClinicalWorks OB module, or a generic HL7 v2
PMS. Solo OB practices and multi-location women's health groups both work.
## What this skill does
1. ACOG prenatal cadence: every 4 weeks until 28, every 2 weeks 28-36, weekly 36 to delivery
2. Screen-window outreach for NIPT (10-13 weeks), first-trimester screen (11-13+6), anatomy scan (18-22 weeks), 1-hour GTT (24-28 weeks), GBS swab (35-37 weeks)
3. Postpartum 6-week visit recall plus EPDS or PHQ-9 mood screen
4. Well-woman annual recall keyed to last cervical cancer screening (Pap or HPV co-test) and last mammogram per ACOG and USPSTF
5. Tdap-in-pregnancy recall at 27 to 36 weeks and influenza-in-pregnancy outreach in season

## Workflow: prenatal cadence
When a new OB is confirmed, the agent reads the LMP or ultrasound-confirmed EDD and schedules the ACOG prenatal cadence forward. For each upcoming visit the agent:
1. Computes the gestational age at the visit
2. Determines which screens or labs open in that window (NIPT, FTS, anatomy, GTT, GBS, anti-D for Rh-negative patients, Tdap, flu in season)
3. Drafts the appointment confirmation that names the visit purpose and any screen scheduling needed
4. Routes to the OB coordinator for approval
The skill handles the reduced-visit prenatal schedule when configured for low-risk patients, and supports the high-risk MFM co-management cadence with separate visits for the OB and the MFM consultant.
## Workflow: screen-window outreach
The screen windows are tight. The skill knows them:

When a patient enters a window the agent drafts a scheduling message that names the specific screen and explains the timing window. Out-of-window screens are flagged for clinical review rather than auto-rescheduled.
## Workflow: postpartum and well-woman recall
After delivery is logged the agent enters the postpartum recall queue:
- Day 1 to 7: passive standby, no outreach unless flagged
- Week 3: scheduling outreach for the 6-week postpartum visit
- Week 6: visit reminder with the EPDS or PHQ-9 attached as a portal pre-fill
- Week 8: if no visit completed, soft re-engagement message
- Month 6: contraception check-in and routing to well-woman if appropriate
For well-woman annual recall, the skill maintains a per-patient ledger of last Pap, last HPV co-test, last mammogram, last bone density, last colonoscopy referral, and drafts the annual visit recall message naming the screens due that year per the current ACOG and USPSTF guidance.

Write-back to the PMS starts disabled. The OB coordinator commits any PMS changes manually for the first two weeks, then write access can be flipped on for low-risk fields (recall date stamps, communication log entries).
## Compliance notes
HIPAA: by default the skill operates on patient identifiers, gestational age, EDD, and appointment metadata. Lab results, ultrasound findings, and chart notes are not put into outbound SMS. For practices that want tighter PHI handling, set phi_mode to identifier_only and the agent will tokenize patient names, forcing the OB coordinator to complete personalized content from inside the secure portal.
Abnormal screens, fetal concerns, and postpartum mood flags are routed to a clinical staff member by default. The skill will never auto-handle communication for a result that is abnormal, indeterminate, or out-of-window.
Confirm your BAA covers the cloud AI provider you point OpenClaw at. The skill itself is local; the model inference is not.

Default is the standard ACOG schedule (every 4 weeks until 28, every 2 weeks 28-36, weekly thereafter). Many practices run a reduced-visit prenatal cadence for low-risk patients, so the config block has a reduced_visit_schedule option. The skill also supports high-risk MFM co-management cadences.
What about EPDS or PHQ-9 for postpartum depression screening?

ACOG recommends a postpartum mood screen at the 4-6 week postpartum visit. The skill drafts the visit invitation with the screen attached as a portal pre-fill (where the PMS supports it). Any positive screen is routed to the clinical team, never auto-handled.
Can I modify this skill?

Yes. MIT licensed. Edit the cadences, the screen timing windows, the message templates, the postpartum follow-up logic. Most practices fork it within the first month to match their patient-communication voice.
Does this replace the OB coordinator?

No. The OB coordinator is the highest-leverage person in a prenatal panel.
